Is there a specific dog you would like to adopt?

IF YES, what is the name of the dog you are applying to adopt?

Before we proceed, please read through and acknowledge you understand the responsibility you are making in adopting a dog and that you confirm the following.

Did you know that all dogs, including rescue dogs, take time to settle in a new home environment? In fact, it can take up to 6 months before your new dog begins to feel comfortable and start trusting in you and the environment. During this time, your dog will most likely go through several transitional phases.

Are you prepared to let the dog settle into home and let the dog go at its own pace? Please tell us in your own words how you will help your dog settle?

HAVE YOUR CHILDREN BEEN AROUND DOGS BEFORE?

It is your responsibility to ensure that both the dog and children are kept safe. The children will need to be taught how to behave around the dog. For example; letting the dog eat in peace, no pulling tails, no over excitement in the first 3 months, no running at the dog and no playing at head level with the dog unattended.

Do you have any other pets ?

If yes, what pets do you have?

PET HISTORY

Have you owned a dog before?

Please describe your experience:

SECTION FIVE

TAKING YOUR NEW DOG HOME:

The law states that your dog must be suitably restrained so they cannot disturb the driver of the car. You are expected to put them in a dog crate, pet carrier, or dog harness or use a dog guard for your car’s boot.

We sell car safety harnesses, slip leads and collars. All the proceeds go towards the looking after of the dogs and cost from £35.00 to £60.00 depending on size and type.

Our harnesses are car safe and use the cars own internal seat belts which means you are not relying on any additional universal seat belt clips that quite often can come undone.

Would you like us to fit your dog for a car safety harness?

If, no, how do you plan on taking your new dog home?

Your new dog MUST be walked on a slip lead for at least the first 3 months, whilst your dog settles and learns to trust you.

Your new dog CANNOT be walked on a harness or clip lead fixed to the collar because your new dog may not have experienced traffic, pushchairs, other dogs, buses or cyclists. All of these can terrify your new dog and sometimes spook them enough to make them bolt. A dog can wriggle out of a harness in less than a minute and even a collar.

Therefore, for the dogs own safety it MUST be walked on suitable slip lead. You can purchase slip leads on Amazon or buy from local pet stores. Alternatively, you can purchase one from us for £12.95 – our slip leads double up as figure of 8 lead to stop your dog from pulling.

Do you understand and agree to walking your dog on a slip lead?

SECTION SIX

ONCE YOUR NEW DOG IS HOME:

ALL new dogs require time to settle and decompress in their new home.

Remember, that your new dog may not have heard a TV, washing machine, iron, kettle and these normal household noises may frighten them.

Please allow your new rescue dog to decompress and get used to its new surroundings without overcrowding it. Please allow your dog to investigate its new surroundings and be prepared for the odd accident.

Where will your dog be kept during the day?

Where will your dog be kept at night?

SECTION SEVEN

TRAINING YOUR DOG:

Your dog will most likely NOT be the perfect pet upon first arrival. It will take time for the dog to settle and readjust into its new surroundings.

It is most likely that the dog will need training. We advise taking your new dog to training classes. This will help with socialisation and behaviour. Are you prepared to undertake dog training classes if needed?

In some circumstances, it may be necessary to seek the help and guidance form a trained behaviourist. Are you prepared to do this if necessary?

Where will you walk the dog?

How often will you walk the dog?

When you collect your dog, we will run through its vaccination and worming history with you. Are you happy to maintain your new dogs vaccinations?

Which veterinary practice will you be using, as they will need copies of all vet history
